By filling out this form, I will receive a timely call back from a fur'endly Bark-A-Bout receptionist who will officially enroll my dog as an Unleash'd Member. Once enrolled, my dog can immediately utilize exclusive benefits reserved only for elite Unleash'd Members.

I understand that I will need to put an active credit card on file, which will be billed every month for $25 until I cancel my dog's Unleash'd Membership.

In order to cancel my dog's Unleash'd Membership, I must fill out the online Membership Cancellation Form, or call up Bark-A-Bout and speak to a fur'endly receptionist.

I understand I can cancel my dog's membership at any time. However, I will not receive a refund for any unused days and my dog will no longer receive Membership upgrades.
